Insurance

  • My practice is in network with all Blue Cross Blue Shield PPO plans.

  • I accept Auto Insurance, Workers' Compensation, and JW HealthCare Support.

  • I accept out-of-network insurance plans*

  • I submit claims to all of the above directly.

  • I am not a Medicare provider, and they will not reimburse for treatment at my practice.

  • Please consider booking Concierge Care appointments if insurance is not an option.

​*Out-of-network Physical Therapy:  If you have a commercial health insurance that is not listed above (i.e. Aetna, Cigna, Humana, United Health Care, etc), I would be considered an "out-of-network" provider. In this case, you would be responsible for paying my current "Out-of-network Physical Therapy" rate at the time of each appointment (see Appointments page), and then you will be refunded the amount your insurance reimburses me.

 

 

Whether in-network or out-of-network, if you would like to know what your financial responsibility will be before you begin care, please contact me, and provide your health insurance Member ID and your date of birth.

I also offer Concierge Care appointments. Accepted forms of payment include cash, check, credit, debit, HSA card, FSA card, and Zelle, and the rates are displayed in the booking widget on the Appointments page.
